Eligibility for Clinic Care. 

Screening for patient eligibility is done by appointment and as openings are available. Please call our office at # (850) 837 - 8424, follow the prompts to "Becoming a Patient," and leave a detailed message. New applicants to HMC will be contacted when they have been scheduled for a screening date. 

1.  Screening:

• Screening is the process of becoming a patient at HMC. At this first appointment applicants will be asked to complete a basic information packet and review their medical concerns with our staff. Collecting this information allows us to qualify applicants for our services and prioritize their needs. Based on the information collected during the screening process applicants will be scheduled with doctors and/or clinicians in the future as availability allows (usually within 1 month).

2. Eligibility:

• An uninsured employed adult or family member.

• Household income does not exceed 200% above Federal Poverty Guidelines.

• Resident of Okaloosa or Walton county.

• Provide a working phone number.

3. Documentation Required:

• Photo ID.

• Proof of Okaloosa or Walton county residency (utility stub, mail with address, etc.).

• Income stubs for previous four (4) weeks.
